Veterinary Consent Form

Please download a copy of our Veterinary Consent Form here and take this to your Veterinary Surgeon for them to sign.

You will need to fill in the sections labelled 'Owner Details' and 'Pet Details' and leave the 'Veterinary Details' for your Vet to fill in. All dogs will require a Veterinary Surgeon to confirm they are fit and well enough for hydrotherapy, even if they don't have any medical conditions. A Registered Canine Hydrotherapist should never swim your dog without Veterinary consent. You can also find the centre terms and conditions on this document.
